Cancellation Policy For Surgery
We have set aside a significant amount of time in our schedule for your surgical procedure; this appointment has been reserved for you. To change or cancel your surgical appointment, a 5-day notice is required to avoid a cancellation fee of $200.
Insurance and Billing
We can file most traditional insurance plans. Please contact our office to verify acceptance of your plan. Petra I. Mayer, DDS is a Delta Dental Premier Provider. We will be happy to file your insurance claims for you.
Payment Options
